Is there an app or service that lets my clients pay me in USD but I receive crypto? Like how a debit or credit card changes currency automatically when you make a transaction in another country.

USD payments (except cash) can be reversed. Crypto payments act like cash and can’t be reversed. What you’re looking for could attract fraud – the USD sender could reverse their transaction while you keep the crypto.
Crypto works best without middlemen, as long as both sides accept it. If your clients use crypto, then you can receive it.
You can also use cash to buy Bitcoin at a Bitcoin ATM. This works well but can be costly. It’s usually better for the sender to buy the crypto themselves and send it from their wallet to yours. ATMs often have high fees, around 15% or sometimes up to 26%.
I’m not sure if there are any apps that do exactly what you want, without you converting USD yourself like with PayPal or Cash App.
But I find it surprising that sex workers haven’t moved to crypto. Two years ago, a lot of the industry almost fell apart when big sites were thinking of stopping adult content, especially places like OF and others that don’t have other business services.

Workers started taking crypto after Pornhub stopped allowing user uploads. Chaturbate has taken Bitcoin for years for users buying CB tips, which is cheaper than using a credit card.
You could use a high-risk merchant like CCBill. They have been working with the adult entertainment industry for decades. There are many others as well.

Just create a random side business like an online candle shop and show that to your bank when they ask about the CCBill deposits.
Back in 2017, there was the Request Network Token (REQ) and it aimed to do just what you want. It was good for merchants but it seems to have faded away since no one talks about it anymore. It’s too bad because it was an interesting idea.
